DESCRIPTION Comunilife has an exciting opportunity for an entrepreneurial and dedicated leader to serve as the Senior Program Director for the Respite program. Comunilife??s Medical/Social Respite Program is a short-term residential program to help patients obtain permanent housing when they no longer require hospitalization but do not have suitable housing in which to complete their recovery. While in the Respite Program, patients are still able to access medical care and other supportive services. The goal of our innovative program is to move patients into permanent housing and to reduce preventable re-hospitalizations and Emergency Room visits. The Medical Respite Program provides Housing Placement Plans which lead to placement in appropriate housing following program discharge, including case management services with arrangements for follow-up medical care and social service assistance with transportation to follow-up medical appointments. Clients are provided temporary short term housing in a safe and sanitary, smoke-free room at the Respite program with 24-hour supervision and monitoring, building security and housekeeping during their brief stay of less than 90 days. Clients are also provided personal hygiene items and access to laundry facilities. Duties: DUTIES Under the supervision of the Senior Vice President of Programs, the Senior Program Director will lead the overall operation of our medical/social Respite service program, which includes management, planning, intake, development, implementation, monitoring, evaluation of policies and implementing proper procedures for the program. The Senior Respite Director has the direct administrative oversight for the day-to-day operations of the Respite Program. Supervises social services staff that includes Case Managers, Housing Specialists and Peer Specialists and handles all Respite Program functions including assurance of case management documentation, external networking/ communication with contracted entities (hospitals and Managed Care Organizations). Reviews and manages all referrals and conducts screenings and intakes to ensure that Respite clients receive appropriate assessment in order to determine appropriateness to the program and readiness for next-step housing; ensures adequacy of the development and provision of Housing Support Plans and case management services to enable clients to successfully move to permanent housing/independent living. Participates as an active member of the agency??s Quality Improvement committee; responsible for ensuring the accuracy and consistency of all information entered in client case files (AWARDS), efficiently manages incident investigation and reporting; develops plans and monitors health and safety of high risk clients including maintaining monitoring plans for medication and wellness checks. Manager all appropriate administrative functions, including budget management. Ensures that all rooms, beds and furnishings are in a habitable and sanitary condition and Conducts routine inspections of the Respites. Ensure 100% bed utilization and supervises staff to ensure that clients are successfully discharged within 90 days into permanent housing where they are prepared to remain without recidivism. Manages compliance of all funder and health and safety standards for the Respite program??s multiple sites Responsible for data collection, analyzing, documenting and reporting on program outputs, outcomes and quality indicators into AWARDS and Salesforce; dashboards, etc. Ensures that Housing Placement Support Plans appropriately prepare client for permanent housing by identifying client needs and coordinating the identified levels of support required to place client into housing. Develop and implement strengths-based assessments and client centered Housing Placement Support Plans with individual clients based on their interests, skills and goals to support them. Develops and maintains client medical and case management reports in compliance with confidentiality and HIPAA requirements. Maintains policies and procedures that comply with all funding and regulatory requirements, guidelines, and directives. Supervises program staff and coordinates staff training, meetings, supervision and ensure other opportunities for the professional development of staff. Manages all program supplied and equipment, spend in accordance with available budgeted funds, maintain and reconcile Petty Cash and Metro Cards regularly and in accordance with agency policy and procedures. Leads in the development and maintenance of systems for managing, monitoring and measuring the quality and quantity of services delivered, the efficiency and effectiveness of staff and the programs. Develop and implement strategies, tools, and tactics to improve the quality and quantity of services while building the reach and reputation of the program. Builds and maintains strong networking relationships with Housing Development Community Based Organizations to facilitate housing referrals and placements and will be the liaison to contracted entities; marketing the program to potential referral sources by describing its services and successes. Qualifications: EDUCATION and EXPERIENCE Master??s degree in social work, nursing, public health, behavioral health related field. Seven years?? experience in homeless services, rapid rehousing, hospital administration, behavioral health or related field. Experience in homeless shelters is preferred. Two year of proven success as a supervisor in a housing or homeless services, medical or behavioral health setting or an appropriate equivalent. Experience with housing placement strongly preferred. Generalized background/knowledge of residential care for persons with chronic illness; both medically and behaviorally is preferred as is knowledge of behavioral health practices and procedures; psychosocial rehabilitation; psycho-education; chemical dependency, motivational enhancement strategies; assessment, crisis intervention, process facilitation and supportive counseling and case management activities. KNOWLEDGE and ABILITIES: Strong leadership skills. Excellent critical thinking and problem-solving skills. Strong documented knowledge of the New York City housing system. Knowledge of NYC hospitals and other potential referrals sources. Strong communications skills (written and verbal). Ability to manage and organize multiple sites in a busy, high volume, time-sensitive working environment. Ability to effectively plan and manage multiple projects and deadlines with competing priorities requiring the ability to adapt strategy to changing conditions and communicate changes effectively. Proficiency with MS Office and Google product suites.
